The TV Diary
Instructions on how to fill out the TV Diary are located on the inside front cover and first page of the diary. The diary is to be filled out with respect to only your television viewing, not the viewing of the entire family. You will be recording your TV viewing for seven consecutive days.

When you have completed the TV Diary, go through the checklist printed on the inside back cover of the diary. Once you are sure that everything has been taken care of, seal it and mail it. To seal it pull off the self-peeling sticker and stick the flap on the back page to the front cover. If you need to, it is okay to tape the flap to the front cover. Make sure that the address on the outside back cover is showing. No envelope or postage is needed.
